TAXES AND SURCHARGES MATRIX
Some or all of the following charges may apply to your calling plan. The charges described in this Section are in
addition to all other rates. With the exception of some of the charges in subsection 6, the charges enumerated below
are not taxes or otherwise required by the government.
1. Gross Receipts Tax Surcharge
A customer residing in a jurisdictions listed below will be assessed a Gross Receipts Tax Surcharge (GRTS).
The GRTS will be assessed as a percentage of the customer's monthly interstate and international charges.

3. South Carolina Universal Service Charge
Sprint will assess a charge to support universal service in South Carolina. The charge, which is
prescribed by the Public Service Commission of South Carolina, is a percentage of the intrastate
and interstate charges within the state of South Carolina of customers whose service address is
in South Carolina. The charge is also applied to all intrastate South Carolina FONCARD
charges.
4. Carrier Universal Service Charge (CUSC)
Effective July 1, 2004, Sprint will apply the FCC-prescribed universal service contribution rate of 8.9%.
The CUSC is assessed on all interstate and international charges (including usage and non-usage) each
month, excluding charges for calls that both originate and terminate outside the U.S.
5. Carrier Cost Recovery Charge
Customers will be assessed a $0.99 Carrier Cost Recovery Charge each month. This charge will help
Sprint recover various costs, including the costs of administering relay services for deaf and hard-of-
hearing consumers, the North American Numbering Plan, other regulatory compliance items, and certain
property taxes.
6. Other Taxes and Surcharges
A customer must pay all excise, sales, use, gross receipts or other similar taxes levied by a governing
body or bodies for services, unless the customer provides Sprint with proof that they are exempt from
paying those taxes. The rates for some of these taxes are set by the governing bodies, while others are
set by Sprint in accordance with the amounts it must pay to the governing bodies. Various state taxes
and surcharges may apply to a customer's bill. These taxes and surcharges are set forth in Sprint in-
state tariffs where required by state law.

8. Frequent Flyer Excise Charge
A customer who participate in an Airlines/Sprint Partnership Program, will incur a monthly Frequent Flyer
Excise Charge calculated as follows:
Total Frequent Flyer Miles Earned X $0.0010
For example: 270 total frequent flyer miles earned from Sprint X $0.0010 = $0.27
9. International Mobile Termination Surcharge (IMTS)
Sprint will apply a per minute surcharge to calls made from your home that terminate to foreign mobile
phones in certain countries. A listing of the countries in which the surcharge applies and the specific
surcharge for each country is found at http://www.sprint.com/mobilesurcharge. Customers will receive
notice of increases to the International Mobile Termination Surcharge and of the introduction of new
countries if they made a call, which terminates to a mobile phone within 3 months of the change. If a
customer has not made a call to a mobile phone, they may check the above mentioned web site or call
Customer Service for the current rates.
10. Payphone Surcharge
A customer will incur a per call surcharge of $0.55 for all calls that Sprint identifies as originating from a
Public or Semi-Public Payphone and for which the customer is billed by Sprint. A per call surcharge also
may apply to calls made from a payphone using a prepaid calling card. This surcharge is set forth in the
Prepaid Card matrix found at www.sprint.com/ratesandconditions/, Rates for Residential.
11. Regulatory Programs
A customer must pay additional charges that Sprint may impose to recover amounts Sprint is required by
regulatory or other governmental authorities to collect on behalf of or pay to others in support of statutory
or regulatory programs. These obligations include but may not be limited to the following: state specific
universal service funds, lifeline charges, mandated user fees, telecom/telecommunications and deaf relay
charges, and other federal and state miscellaneous charges assessed against telecommunications
services.
11. Single Bill Fee
If a customer choose to receive their Sprint charges on the bill they receive from their Local Exchange
Carrier, they will be assessed a monthly fee of $1.50. This fee is not assessed if: 1) a customer’s Local
Exchange Carrier is Sprint, 2) a customer’s usage for the month (including any Monthly Recurring
Charge) is greater than $40, or 3) a customer subscribes to an offer where the fee is excluded as noted in
the Service Matrix. This fee will not be prorated for partial month’s usage and does not apply towards the
satisfaction of usage volume requirements. This charge does not apply to blind and visually impaired
customers in Missouri who request invoices in Braille or large print.
