Thank you for participating in this PrepLogic promotion. To receive your practice exam rebate, please
complete and submit this form by postal mail to PrepLogic, along with your original order receipt and
verification of a passing score on the corresponding certification exam. For qualifying submissions,
PrepLogic will provide a rebate check equal to the amount of the practice exam purchased (not including
taxes, fees, shipping and handling or any other charges). Requests for this promotion will be accepted 30
days after the promotion closes on November 7. Requests will not be accepted sooner than 30 days
after the close of the promotion. Requests must be received no later than 60 days after the close of the
promotion. Corresponding exam must be taken and passed within the submission period.

Qualifying Orders
Corresponding exam must be passed in order to be eligible for rebate. Third party verification of passing
score must be provided along with rebate request. For purposes of calculating purchase amount,
purchases will consist of amount paid for qualifying PrepLogic product. Qualifying product includes
single practice exams. Bundled product does not qualify as product for purpose of calculating
promotional incentive purchase amount, discounted, non-discounted or otherwise priced. Rebate
amount will be calculated as the lowest price paid based on dollar amount without regard to retail
pricing or otherwise advertised pricing. The following items are not considered as qualifying purchase
items and as such will not be counted toward purchase amount; VUE vouchers, Prometric Vouchers,
Bundled Products, Exam Packs, Clearance Vouchers. Qualification of purchase products is exclusively at
the discretion of PrepLogic. One promotional incentive per person, household or otherwise stated or
implied commonality. Purchase does not guarantee qualification for promotional incentive. Promotional
incentives are not eligible for previous, ongoing or recurring purchases and payments. Previous, ongoing
or recurring purchases and payments not eligible toward qualifying purchase amount in regards to
promotional incentive.
Promotional incentive offer will be voided in the occurrence of, but not limited to the occurrence of
chargeback, return, cancellation, NSF notification or payment declination. Promotional incentive offer
may be voided due to additional unforeseen occurrences in regards to payment, fulfillment or validation
of promotional incentive purchase. Promotional incentive offer void where taxed, restricted or
prohibited by law. Promotional incentive offer is non-transferrable. PrepLogic is not responsible for
Promotional incentive offer fulfillment shipments unable to be delivered due to technical issues beyond
control of PrepLogic, nor is PrepLogic responsible for submissions received outside of established
request submission period due to technical issues beyond the control of PrepLogic, including but not
limited to postal or other delivery method failure, improper or incomplete labeling or addressing,
insufficient postage or other undisclosed or unforeseen reason. Promotional incentive offer is not valid
with prior purchases, clearance merchandise, bundles or with any other offer. No credit if offer is
refused. Void where prohibited, taxed or restricted. Return or chargeback of merchandise shall
invalidate offer. Allow 4-6 weeks for processing of promotional incentive offer request submission.
